Ciri didn't not know that for the last 2 jugs she will receive a special treatment. At a hand sign, one of the guards grabs her by the torso with one hand while supporting her pelvis with the other, lifting her while the other two position a trestle under her legs. When she's released, the trestle will push her belly upward, extending further her body and her agony.

Only when positioned like that she will get her last two jugs of water.

The eigth and last jug of water is administered. The slender figure of Ciri, now horribly distended by 16 pints of cold water and sinking in the sharp trestle resembles a hideous, grotesque pregnant woman from a derranged nightmare. At least, the water ordeal is over.

I know you don't like overly enthusiastic praise, but I have to say that this image is close to perfect and very much a turn-on for this old sadist. It's exactly as I envisioned the scene when I first read that account of her torture in the book.
For me, you could not have picked a better scene to illustrate, or done any better with it.
