Foxy Slave does a great job portraying fear/avoidance/pain during the "hot iron" segment of Cruel World's Nightmare Awakening 4 (a future release). There is smoke rising during the "burning", you just can't really see it well in this still.

Also, this video is fantasy (nightmare), so Alex is dressed appropriately, with a black mask and camo utilities/combat boots under that monk's garb. Just a little different and thought you should be aware in case you were expecting a straight medieval scenario.
