I think you're seeing several things at work simultaneouslty to make up this nice image:
The adhesive pads were usually attached to a TENS unit: https://my.clevelandclinic.org/health/t ... ation-tens that in this case delivered a current across (through?) her nipples.
The application of the TENS unit could very well be painful, and (in medical therapy sessions) was usually turned up to the highest level a patient could stand. In very extreme examples, the muscles are "jumping" under the skin.

Her pussy is filled with a dildo that is "wired" up to look as if it were part of the TENS. I think the strings along the upper part of her thighs are to hold the dildo in place.

Thanks for the polite reply. You meant "The Box" as I sometimes liked to call it in my ignorance.

I really liked that device, as it implied the GIMPs wrists were fastened behind her. And her knees were spread wide and her ankles were fastened. A very helpless woman, just the way I like them!
But I speculate that her wrists weren't tied/fastened to anything, and that she was actually using her hands to brace against a support in the back of the box.
