Yep, that's a fine point that sometimes is overlooked by the newbie GIMPer. Actually stretched tautly (or to "torture" specs) is a nice detail. You can sometimes tell by if her hands become discolored (purplish) by the strain on her arms/wrists/body. Just another detail. And different girls/women have different tolerances.

Another cool thing that Ogre (Dungeoncorp) used to do with his racked girls, was lift them under the waist to make sure there wasn't any "stiction", and that they were fully stretched.

When you're stretching someone slowly, stiction (between her skin and the rack surface) can interfere with the extent of the stretching. This also may have prevented injury by allowing for a more gradual, linear stretching of the ligaments, tendons and joints.
I don't think this would be so much a problem with the wood surfaces found on medieval racks, though. Just the newer leatherette type surfaces.
After all, this is fantasy kink.
