Great question! The bondage FILMS, and I mean 12 frames per second films, those were shot in the 1970s and 1980s. Penetration was not allowed, of any type. No dildo could be seen entering her vagina. No penis could be filmed entering a vagina. Films that showed that were considered "Illegal Porn" which brings in Federal officers. So no, there was NO penetration in HOM films.
American law was relaxed in 2002 or 2003, and penetration was finally allowed. That is what current, modern porn producers use. But when HOM was in business, there was NO penetration allowed. None.

As has already been stated there was no penetration allowed in the US. If I recall correctly, the nurse clip you mention was made for the Erotic Perversion series that was marketed in Europe, hence the explicit scene at the end. I also seem to recall a Blakemore interview where he stated that for some time it was not even allowed to remove the panties and show female genitalia.
